We encourage the use of a CLAIM NOTIFICATION form, but please note the following information must be included for potential claims:
- Reasons for your decision to report this as a potential claim; and
- Details with dates of the alleged act that gave rise to the potential claim; and
- Possible amount of injuries or damages arising from the potential claim;
- The names of potential claimants; and
- The manner in which you first became aware of the specific potential claim.
